Our Mission is to promote the dignity, life and spirituality of older people through connected and inclusive communities.We are looking for passionate and caring individuals to provide intensive support and assistance to vulnerable older Australians to understand and access aged care services as well as advocate for those unable to access aged care services without support across South East Sydney.About the Role:Identifying individuals in need of aged care services through Assertive Outreach and developing relationships with them so support is accepted and actioned.
