The role role is responsible for: Assist people to remain living at home for as long as possible through enabling independence and supporting their care needs; Enable clients to have a choice and flexibility in the way that care and support is provided at home; Maintain caring relationships with care recipients by providing services based on the signed agreement; Perform all duties with professionalism and care; Perform care and support services in accordance with client care plans in a timely and effective way taking into account the clients abilities, preferences and decisions Assist the clients with daily living activities and or personal care according to their care needs and care plan ensuring their independence and dignity is maintained Perform light domestic household duties including but not limited to meal preparation, cooking, feeding washing dishes or laundry Perform general household cleaning including but not limited to dusting, mopping, ironing, vacuuming, cleaning toilet & bathing areas Assist toileting of clients including transferring clients using appropriate equipment Accompany clients on social outings, client services, medical appointments and shopping ensuring safety of the clients Provide companionship and emotional support to client Monitor client progress and changes, report any changes to client condition or environment in a timely manner Document and record information accurately and in a timely manner Maintain the privacy & confidentiality of client at all times Comply with organisational, policies, and procedure as implemented and enforced Carry out all reasonable and lawful instructions by your Supervisor and maintain skills and knowledge in relation to your work including attending staff and supervision meetings Attend and complete all annual mandatory and compulsory training or refresher courses to make sure you comply with the legislative requirements of your role Act as a constructive member of the Operations team Proactively be aware of safe working conditions and follow safe work practices in all areas and report all incidents and potential hazards in a timely manner Maintain a high standard of conduct and work performance and model Melbars Values to promote the organisations reputation with internal and external stakeholders Role Key Performance Indicators Consistently comply with company policy and procedures including incident reporting, care delivery, manual handling, risks and hazard Consistently demonstrate duty of care to clients while advocating for their independence, dignity and choice Consistently providing quality and professional customer service Client engagement and feedback are consistently positive and indicate respect for the service and good working relationship Client indicate a high level of satisfaction with the service Quality standards and regulatory compliance are met; All compulsory and mandatory training are completed on time Consumer incidents/hazards/injuries reported in a timely manner Professional Behaviour and Conduct aligns with Melbar Values and consistently and effectively modelled in all work interactions QUALIFICATIONS Essential: Certificate III in Aged Care/ Disabilities/ Individual Support is essential; A current First Aid Certificate is required; A valid Australian drivers license Own reliable and roadworthy car with current comprehensive insurance A National Police Criminal Record check from within the last 12 months.
