The adult mental health service components are: The Footbridge Community Care Unit (CCU), the 44 bed Acute Inpatient Service (AIS) located on the St. Vincents Hospital site, the North Fitzroy PARC and two community mental health services (CMHS) at Hawthorn and East Melbourne (Clarendon CMHS). Community mental health services comprise the Triage and Crisis assessment and Treatment (CAT) Service and the Primary Intervention Care Team (PICT) includes Extended Triage and HOPE (Suicide Prevention), Continuing Care (CCT), Mobile Support and Treatment (MST) and Homeless Outreach (CHOPS) services. A Consultation & Liaison Service is provided to the health service. NEXUS Dual Diagnosis Service, the Body Image and Eating Disorders Assessment and Treatment Service (BETRS) and NEVIL Training Cluster are regional services. Statewide services are the Victorian Dual Disability Service and the Victorian Transcultural Mental Health. All SVMH clinical staff are expected to be familiar, and practice in a manner consistent with the National Practice Standards for the Mental Health Workforce.POSITION PURPOSEThe Mobile Support and Treatment Service provides intensive home and community-based recovery oriented services, seven days a week, extended hours, for people with a significant mental illness and associated high-level disability needs who require additional clinical support than is available through continuing care services. The multidisciplinary team provides a range of clinical and therapeutic interventions using the recovery-focused Strengths Model of Recovery-Oriented Practice. This position will be required to work a rotating roster over seven days a week and work extended hours.Under the guidance of the Team Manager and Team Consultant Psychiatrist, and in conjunction with team members the Enhanced Care Clinician will provide and coordinate short to medium term additional specialist, targeted therapeutic interventions to consumers that are receiving case management services. Interventions provided will be aimed at supporting psychosocial recovery, fostering individualised recovery, independence, and improved health outcomes for consumers that have been identified as having severe and complex needs. Interventions provided could include, but not be limited to, cognitive behavioural therapy, family therapy, peer support, interventions focused on activity/vocation or additional treatment interventions to meet physical health or drug and/or alcohol needs.POSITION DUTIESWork within the Strengths Model of recovery orientated practice.Build a therapeutic alliance with the consumer and family/carers and actively engage them in the planning, delivery and evaluation of their care.Actively participate in daily handovers/clinical meetings that determine day to day clinical priorities for the team.Undertake the role of Duty Clinician and Code Grey Coordinator as requiredIn collaboration with the treating team, undertake the assessment, coordination and provision of treatment and delivery of targeted evidence-based interventions to consumers with complex mental health related needs and their family/carers in line with SVMH policies and strategic direction.Utilise discipline-specific skills as appropriate and as required by consumers, their family/carers, and the Service.Assist in the provision of primary, secondary and tertiary consultation services, community development and education, liaison and linkage to other agencies that also provide support to people with mental illnesses as required.
