The adult mental health service components are: The Footbridge Community Care Unit (CCU), the 44 bed Acute Inpatient Service (AIS) located on the St. Vincents Hospital site, the North Fitzroy PARC and two community mental health services (CMHS) at Hawthorn and East Melbourne (Clarendon CMHS). Community mental health services comprise the Triage and Crisis assessment and Treatment (CAT) Service and the Primary Intervention Care Team (PICT) includes Extended Triage and HOPE (Suicide Prevention), Continuing Care (CCT), Mobile Support and Treatment (MST) and Homeless Outreach (CHOPS) services. A Consultation & Liaison Service is provided to the health service. NEXUS Dual Diagnosis Service, the Body Image and Eating Disorders Assessment and Treatment Service (BETRS) and NEVIL Training Cluster are regional services. Statewide services are the Victorian Dual Disability Service and the Victorian Transcultural Mental Health. The Acute Inpatient Service includes 5 beds designated as the Koori State-wide Inpatient Service and linked to the Victorian Aboriginal Health Service.St Vincents Mental Health is committed the recruitment and retention of high-quality staff. The service aims to support staff to maintain and develop their skills to provide excellent clinical care and to meet their own personal and career goals. SVMH has a strong education and research focus in addition to affiliations with a number of universities and educational institutions. All SVMH clinical staff are expected to be familiar, and practice in a manner consistent with the National Practice Standards for the Mental Health Workforce.POSITION PURPOSEThe Mobile Support and Treatment Service provides intensive home and community-based recovery oriented services, seven days a week, extended hours, for people with a significant mental illness and associated high-level disability needs who require additional clinical support than is available through continuing care services. The multidisciplinary team provides a range of clinical and therapeutic interventions using the recovery-focused Strengths Model of Recovery-Oriented Practice. This position will be required to work a rotating roster over seven days a week and work extended hours.Under the guidance of the Team Manager and Team Consultant Psychiatrist, and in conjunction with team members the Enhanced Care Clinician will provide and coordinate short to medium term additional specialist, targeted therapeutic interventions to consumers that are receiving case management services. Interventions provided will be aimed at supporting psychosocial recovery, fostering individualised recovery, independence, and improved health outcomes for consumers that have been identified as having severe and complex needs. Interventions provided could include, but not be limited to, cognitive behavioural therapy, family therapy, peer support, interventions focused on activity/vocation or additional treatment interventions to meet physical health or drug and/or alcohol needs.POSITION DUTIESWork within the Strengths Model of recovery orientated practice.Build a therapeutic alliance with the consumer and family/carers and actively engage them in the planning, delivery and evaluation of their care.Actively participate in daily handovers/clinical meetings that determine day to day clinical priorities for the team.Undertake the role of Duty Clinician and Code Grey Coordinator as requiredIn collaboration with the treating team, undertake the assessment, coordination and provision of treatment and delivery of targeted evidence-based interventions to consumers with complex mental health related needs and their family/carers in line with SVMH policies and strategic direction.Utilise discipline-specific skills as appropriate and as required by consumers, their family/carers, and the Service.Assist in the provision of primary, secondary and tertiary consultation services, community development and education, liaison and linkage to other agencies that also provide support to people with mental illnesses as required. This includes participating in and contributing to locally held portfolios.Display active participation in professional development and ongoing learning opportunities.Provide case management to cover secondary case-loads for other case managers when required.Attend and participate in discipline specific meetings and display a commitment to attending discipline specific training.Participate in Line Management regularly with the team managerDisplay a high standard of clinical practice and professional conduct at all times.Work collaboratively within the multi-disciplinary team.Ensure that all reporting information is coordinated and completed as required.Support the work of other partners in care, MHCSS and primary care providers and ensure their participation in the consumers clinical care.Provide professional supervision to staff and to students on placement as regulated within the discipline (this does not apply to Psychologists with classification P2).Maintain all documentation in line with SVMH policies, including clinical documentation, outcome measures and CMI reporting.SELECTION CRITERIAEssential Registration, License Or Qualification RequirementsA minimum of at 2 years post qualification clinical experience within Mental HealthRegistration with Australian Health Practitioner Regulation Agency (all disciplines except Social Work).Social Workers - An approved degree in Social Work and eligibility for membership of the Australian Association of Social WorkersCurrent Victorian drivers licenseOther Essential RequirementsClinical experience within Mental Health Services.Experience in the provision of assessment, discipline specific intervention and case management services for people with serious mental illness and complex needs and their carers.Demonstrated ability in working as part of the multi-disciplinary clinical team in promoting a standard of excellence in clinical practice.
