Add expected salary to your profile for insightsRecruiting for a full time Care CoordinatorWe are currently looking for a full-time Home Care Coordinator to join our Sunshine Coast team Monday to Friday, 8.30am 4.30pm. You will be joining a growing national network where you can make a difference and practice your skillset and values in a positive, friendly and flexible work environment, working in Home Care community.The Case Manager/Care Partner performs an integral role in assisting in individualised services being delivered to our customers in their homes, ensuring our participants can remain living their optimal lives, safely at home for as long as possible.About us:Across Australia Just Better Care provides in-home aged care and disability support services, supporting people to live independently. For over 20 years, we have been supporting older people, and people living with disability, to live independently in their own homes and stay connected to their local community.Our friendly, compassionate and experienced team makes this possible.We offer:- Use of company vehicle, laptop and mobile phone.Applicants with minimal experience but responsive/ proactive learner within a fast paced working environment are also encouraged to apply.Duties and responsibilities may include:Working with or under the guidance of medical practitioners and Allied Health professionalsCollaborating with clinical and experienced colleagues and a very supportive admin teamEnsuring care plans are monitored and reviewed on a regular and ongoing basis and in response to changes in participant needsEnsuring services are delivered are in line with the person centred and consumer directed principlesMonitoring and evidencing or participants wellbeing and documenting progress of participantsWorking withparticipants tohelpthem participate in the dailyactivitiesthey want to do, including hobbies, their interests, to moving about their home easilyEnsuring that all services are provided in a caring and respectful way in accordance with Just Better Cares policy and procedures, legislative requirements and program specific guidelinesAssist in participant service schedules including service planning, confirmation and short notice placements or cancellationsLiaison with Aged Care Assessment Team (ACAT), My Aged Care (MAC) Portal, PRODA and other regulatory bodiesAssist participants to access other alternative services where the services are no longer meeting their needsUndertake administrative duties including data entry, processing invoices, following up participant enquiries, ensuring documenting all interactions ensuring complianceetc.Ensuring evidence is gathered and uploaded including quotes, confirmation of delivery of services etc.Liaise with relevant community based agencies and service providers to ensure an integrated and cohesive approach to service delivery and to ensure standards are met in accordance with program objectivesWork with participants with their budgets and monitorparticipants budgets in accordance with program guidelines.Communication with key stakeholders, internally and externally.Community Engagement:- engagement with key community stakeholders, promoting Just Better CareIdentify and participate in personal professional development.Apply and uphold principles of equity and anti-discrimination in the workplace and adhere to organisational and legislative Health, Safety and Environment requirementsAuditing of participant files regularly to ensure complianceConducting skills assessments of Community Support Professionals who perform complex care services as requiredDemonstrating a commitment to best practice and evidence of continued professional developmentParticipating in staff meetings and attending training opportunities when requiredAny other duties as required to meet the ongoing needs of the organisationWhat you will need:Strong administrative, computer, time management and organisational skillsStrong communication skills and outstanding Customer serviceKeen eye for detail, impressive follow up and tracking/ auditing methodologyCurrent CPR / FirstAid CertificateCurrent and satisfactory Criminal Record CheckCurrent and satisfactory Federal Police Check, NDIS Workers Screening Card& Blue CardCurrent Australian Drivers LicenceReliable, registered and insured vehicle (comprehensive insurance is preferred)Previous experience in the community care or health/aged care sectorPrevious experience working with home care ComplianceUnderstanding of Community Nursing guidelines or previous community nursing experienceCert IV in Workplace Training & AssessmentCert III in Individual SupportNext steps:If a fast paced working environment with variety and challenges is what you are looking for, to showcase and exhibit your talents, contact: hrsc@justbettercare.comUnlock job insightsSalary match Number of applicants Skills matchCommunity Services & Development 1,001-5,000 employeesFor over 15 years, Just Better Care has been supporting older people, and people living with disability, to live independently at home and stay connected to their local communities.
