Offering 47 high-care suites and 111 assisted living apartments. HyeCare is innovating beyond traditional aged care models. We need a Quality & Education Manager to ensure excellence in care, governance, and team capability while meeting the strengthened Aged Care Quality Standards.Discover the project here: more about us: OverviewAs the Home Health Care Coordinator, you will play a vital leadership role in delivering community-based aged care services. You will coordinate the end-to-end case management process, oversee staff performance, manage consumer budgets, and ensure compliance with aged care standards and legislative requirements. This is a full-time opportunity reporting to the Director of Nursing.Key Responsibilities Lead and mentor a multidisciplinary health team to deliver high-quality home care. Manage the admission process, case conferences, and ongoing consumer care agreements. Coordinate care plans and budget management for Home Care Package clients. Ensure compliance with aged care quality standards, WHS, and continuous improvement processes. Actively collaborate with internal teams, subcontracted services, and external stakeholders. Contribute to policy development, risk management, and organisational growth.What Were Looking ForTertiary qualifications in Community Care, Case Management, or Health/Welfare.Minimum 5 years'' experience in an aged care or case management manager role.Postgraduate qualifications in Health or Management (desirable).Strong understanding of aged care legislation, quality standards, and compliance.Proven leadership, communication, and stakeholder engagement skills.Demonstrated experience in budget management, rostering, and care coordination.Commitment to cultural sensitivity, inclusivity, and consumer rights.Skills That Set You ApartStrategic thinking and a solution-focused mindset.Ability to mentor and build high-performing care teams.Competence in using digital care systems and tools (e.g., ECMS).Strong organisational and time management skills.Why Join Us?At Hyecorp and HyeCare, we dont just build and operate properties we create exceptional living experiences.
