We need a Quality & Education Manager to ensure excellence in care, governance, and team capability while meeting the strengthened Aged Care Quality Standards.Discover the project here: more about us: OverviewAs the Home Health Care Coordinator, you will play a vital leadership role in delivering community-based aged care services. You will coordinate the end-to-end case management process, oversee staff performance, manage consumer budgets, and ensure compliance with aged care standards and legislative requirements. This is a full-time opportunity reporting to the Director of Nursing.Key Responsibilities Lead and mentor a multidisciplinary health team to deliver high-quality home care. Manage the admission process, case conferences, and ongoing consumer care agreements. Coordinate care plans and budget management for Home Care Package clients. Ensure compliance with aged care quality standards, WHS, and continuous improvement processes. Actively collaborate with internal teams, subcontracted services, and external stakeholders.
