This role involves working closely with clients to implement supports outlined in their NDIS plans, fostering capacity building, managing the impact of their disabilities, and enhancing their overall health, well-being, and community participation.Required SkillsGood with people and motivated to take the lead.Building strong relationships with clients and adapting to change.Organized, follows instructions, and values principles.Experience in support work and understanding care requirements.Desired Skills:Achieving work goals and effective communication.Writing and reporting skills.Management experience and driving initiatives.Certificates/diplomas in leadership or management.You will need100 Points of identification.A valid Driver''s license and comprehensively insured motor vehicle.Full Australian working rights.Australia Business Number (ABN).Valid NDIS Worker Screening Check.Relevant States Working With Children''s Check.Public Liability and Professional Indemnity Insurance.Copies of relevant course certification.About YOU:Providing support to clients based on their NDIA plan, implementing all types of supports outlined in their plans, including formal, mainstream, and funded supports.Utilizing a strengths-based approach when collaborating with clients to create a Support Plan, involving not only the client but also relevant stakeholders.Building strong and professional relationships with clients and their families while maintaining appropriate boundaries.Effectively managing and resolving client issues and complaints with respect and fairness in accordance with HAHS policies and procedures.Maintaining a detailed record of all interactions with clients, families, carers, and support services within a centralized database.Prioritizing, coordinating, and efficiently managing a caseload and various tasks in a timely manner.Ensuring that clients have access to information about relevant community resources and making appropriate referrals when necessary.Actively engaging in professional development activities and participating in relevant committees as identified through supervision sessions.Demonstrating proven experience in working with individuals with autism, disabilities, and/or mental illnesses.Assisting clients in understanding and utilizing their plan''s supports, navigating the client portal, and selecting service providers.Empowering clients to enhance their ability to engage with their local community, increase independence, and effectively coordinate their supports, especially during times of crisis.Guiding clients and their support networks in navigating NDIS processes, such as establishing agreements with service providers, managing budgets flexibly, and refining goals, objectives, and strategies.Ensuring financial responsibility by meeting budget requirements, completing forecasts, and providing recommendations for more effective cost and labor management.Implementing risk assessment, mitigation, and quality improvement practices to ensure the highest standards of care and support.To apply for this position with Help at Hand Support, please follow these steps and provide the required documentation:Cover Letter: Include a cover letter that addresses the following selection criteria:Motivation: Explain why you have a strong desire to work as an Independent Contractor for Help at Hand Support.Qualifications: Share your relevant background in Support Coordination or your extensive experience and knowledge within the field of disability support and NDIS.Contribution: Detail your plans and strategies for actively contributing to the growth and enhancement of Help at Hand Support services.
