Job DescriptionCommunity Care Team LeaderReference number: 10688Closing date: 11pm, 16 July 2025Permanent full-time opportunitySalary commencing at Council Band 6, $97,648.13 (Commensurate with experience)Lead Councils in home support team who are vital to the quality of life enjoyed by individuals and families within Moonee Valley and operate as part of a complex service system funded through Commonwealth, State and Local Government.The OpportunityWorking within Councils Young, Inclusive and Ageing Communities, you will be responsible for leading a team of direct / community care staff to ensure the provision of high-quality home and community services in accordance with clients care plans and Commonwealth and State program guidelinesThis roles reports directly to the Coordinator- In Home SupportKey roles and responsibilities include:Provide effective supervision of direct care staff, including annual performance plans, performance monitoring and management, professional development, training, recruitment, return to work, and other human resources / industrial mattersBe onsite to provide supervision and support to direct / community care staff in residents homes for training purposes and during emergency situations.Work with rostering team to ensure the roster and duties of Direct Care Workers and Community Care Workers are in accordance with, but are not limited to, their minimum hour agreements, Moonee Valley Enterprise Agreement and Award requirements, corporate policies and relevant legislationEnsure accurate and timely record keeping by ensuring comprehensive input of case notes, client files and databases, and keeping client data up to dateWork with Care Partners to undertake assessment of client concerns, complaints and/or compliments relating to the provision of service, including escalation to Coordinator Community Care (e.g. quality of work performed, WHS issues) to ensure a satisfactory outcomeRespond promptly to non-response and client incidents in accordance with relevant policy and procedures and escalate to Coordinator Community Care and Care Partner as requiredConsult where appropriate with the Coordinator Community Care and Care Partner, regarding the complex needs and service requirements of people who may be vulnerable or at riskEnsure that Direct Care Workers and Community Care Workers are fully informed of client needs and special requirements in accordance with their care planOrder, store and distribute PPE and uniformsTake part in the On-Call duty roster, coordinating client services and providing support to Direct Care staff after hours and on public holidaysConsistent and constant monitoring and review of the Work Health and Safety of staff, their duties and tasks, regular reviewing of processes and risk assessments.Comprehensive investigations in the reporting of staff and client incidents.Participate in the emergency relief efforts to support recovery during a municipal emergency/ eventYou:Degree or diploma qualifications in the allied health, management/leadership, community services or other relevant fields with demonstrated experience in Aged and Disability Services field or similar role; or a Certificate III in community services or other related field and substantial experience in Aged and Disability Services or similar roleExtensive experience in staff resource management and supervisionDemonstrated highly developed interpersonal skills and ability to respond to difficult situations appropriatelyComprehensive knowledge of Commonwealth and State funded programs and the local service system that provides support and services to older people and people with disabilityDemonstrated understanding of the needs of older people, people with disability, their carers and people from diverse communitiesHow To ApplyApplications must be submitted through our online system My Portal and should be in line with the Moonee Valley City Council Application Guide.
