All services are delivered in a linguistically and culturally appropriate manner with a commitment to enhance individuals capacity and build community capital.What you will be doingIn this role, youll work closely with older adults to coordinate personalised care plans that support their ability to live safely and independently at home. Youll conduct assessments, link clients with appropriate services, and regularly review their needs to ensure quality care. As a key advocate for your clients, youll collaborate with families, service providers, and other professionals to promote wellbeing and dignity. Youll also maintain accurate documentation and ensure compliance with aged care funding and reporting requirements, all while supporting clients from diverse cultural backgrounds with empathy and respect.The role is crucial to the growth and the implementation on strategic growth.Key ResponsibilitiesReceive and review new client referrals via My Aged Care, confirming eligibility for Support at Home servicesEngage clients and families with empathy and cultural sensitivity, clearly explaining services, rights, and the intake processProvide culturally appropriate information and referrals, especially for Greek-speaking and other CALD clientsEnsure informed consent and promote client-directed decision-making throughout the intake and assessment processCoordinate and book initial assessments or appointments, ensuring responsiveness to client needs and timelinesLiaise effectively with My Aged Care, internal care teams, and external providers to ensure a seamless intake experienceConduct preliminary intake assessments via phone, remote, or home visits to evaluate client needs and service suitabilityIdentify and document environmental, physical, or psychosocial risk factors affecting client safety or care provisionDevelop risk mitigation strategies in collaboration with care coordinators and communicate required controlsMaintain accurate, timely, and confidential client records in line with privacy legislation and internal proceduresSupport equitable access for clients with limited mobility, low digital literacy, or complex personal circumstancesEscalate urgent, complex, or high-risk referrals and contribute to case discussions and service planning as requiredRole RequirementsTertiary qualification in Community Services, Social Work, Aged Care, or a related field.Proficiency in My Aged Care systems and intake/referral pathways under the Support at Home program.Bilingual fluency (e.g., Greek and English) preferred to support culturally diverse clientele.Proficiency in the use of Microsoft Office Suite and packages such as Word, Excel (Intermediate Level Mandatory and Advanced preferred) Access, Internet and E-mailMandatory RequirementsDriving LicenseAccess to own vehicleUndergo Criminal History Record Check at the required intervalsNot subject to any Employee Screening or Banning Orders under relevant state or federal legislation (e.g., Worker Screening Act 2020 (Vic)).What we offerEnergetic and Supportive team environment.Award pay rate according to the Social, Community, Home Care & Disability Industry Award 2010, Social and community services employee, Level 4 ($44.58ph $47.97ph)Salary packaging (benefit from free tax earnings up to an annual cap amount of $18,550).Access to EAP.How to applyIf you are interested in applying for this role, please submit your resume and a cover letter via our careers pageFor further information regarding this role, please contact Georgina Meyne from the People and Culture team at hr@pronia.com.auPRONIA is committed to protecting children and young people from harm.
