Be part of something meaningful.About The RoleAs a Clinical Care Coordinator you will play a vital role in delivering high-quality clinical care and fostering a culture of excellence across the home. Working closely with the Home Manager, you will lead and support the care and clinical teams to ensure each resident receives personalised, dignified, and safe care that respects their individual identity, culture, beliefs, and life experiences.This role is ideal for a Registered Nurse with strong clinical experience and with a commitment to continuous improvement, with the ability to coach and mentor staff while contributing to quality and governance activities.Key ResponsibilitiesLead clinical care planning, documentation, and delivery to meet the holistic needs of each residentChampion person-centred care and involve residents and families in meaningful care decisionsGuide and support clinical staff in AN-ACC assessments and accurate, timely documentationSupport and educate Registered and Enrolled Nurses accountable for daily clinical responsibilities, including case reviews and care outcomesConduct regular clinical audits and ensure prompt follow-up on care gaps or concernsSupport a culture of safety, compliance, and continuous improvement across the homeAbout YouAHPRA-registered Registered Nurse (Div 1)Proven leadership experience in aged care or a senior clinical role, with a passion for empowering othersStrong understanding of aged care best practice, including AN-ACC and clinical documentation requirementsExcellent communication, negotiation, and interpersonal skillsConfident with technology and clinical systems (e.g., Word, Excel, clinical software)A team player who leads by example and embraces MACGs values of respect, dignity, and compassionWhat We OfferA supportive, family-oriented culture where your contributions are recognisedOpportunities for ongoing professional development and leadership growthThe chance to shape care in a home where your voice and expertise are valuedA role with real purpose: every day, you make a differenceWhy Join MACG?Medical & Aged Care Group (MACG) is a proudly family-owned and values-driven aged care provider, operating nine residential homes in Victoria with capacities ranging from 49 to 149 beds.
