About the RoleThe purpose of the role is to ensure the best outcomes for our participants/workers by navigating and facilitating the access, management and cohesion of services and supports following discharge from hospital and during their time of adjusting and settling into community life following serious injury. as an Rehabilitation Case Manager you will:Use a person-centred approach, support the participant/worker to develop a My Plan to maximise their involvement in planning through self-assessment, self-reflection and self-identification of strengths, priorities and goals in alignment with KPIs of the role.Guide the participant/worker and their families in setting expectations, understand what they can reasonably expect from Care and Community and to foster a life-long relationship that is built on trust.Assess and actively monitor the effectiveness of selected strategies and supports in meeting goals and steps throughout the My Plan period.Ensure appropriate selection of supports in subsequent plans and to ensure the participant/worker is making future decisions based on comprehensive information about effectiveness of past choices as supported by internal specialist services.Apply critical reasoning and thinking within a decision-making framework within the team and in collaboration with internal specialist services and use of resources when establishing and coordinating requests for services and supports to ensure they appropriately meet the needs of people with severe injuries in terms of their progression along the rehabilitation continuum.Approve recommendations and funding regarding reasonable and necessary treatment, care and support within agreed functional delegationsEngage immediately in the event of a serious incident or risk to the participant/workers safety, assess and implement a plan to ensure the necessary supports are in place to provide the safety and wellbeing of the participant/workers wellbeing.Transition the participant/worker to a less intensive support resource when the participant/worker no longer requires a rehabilitation case management service.Ensure participants/workers understand who they can seek assistance from should their needs change once a case manager has disengaged.Keep abreast of and understand the legislative framework and Guidelines and how these contribute and impact the decision-making framework.Contribute to the knowledge management of research and advice on injury management and service delivery to people with severe injuries to identify gaps and barriers in service delivery and share this broadly across the business in collaboration with internal specialist services.Work within the team to ensure the appropriate provision of services for people with severe injuries in a cost-effective manner.Work collaboratively with all teams across Care and Community, in the design and implementation of internal quality assurance management of complaints and continuous improvement systems to drive quality and consistency in decision making.Maintain relevant records and manage service approvals and payments in accordance with agreed needs, assessments and plans.Liaise and collaborate with service providers to ensure the optimal delivery of programs to people with severe injuries.Identify and participate in professional development opportunities to ensure currency of skills and knowledge in regard to current evidence and best practice.Skills & ExperienceDemonstrate at least 5 years of full time (or equivalent) work experience which has equipped you with transferrable skills to provide case management of people with disabilities similar to those supported by our schemes and/or tertiary qualifications in an allied health, or other relevant, discipline.Available to work 5 days per week.A current drivers licence, with the ability to travel regularly within regional NSW and InterstateMaintain a current Working with Children Check.Up to date with current Public Health Order requirementsHighly developed understanding of the needs and perspectives of people with disabilities.Critical reasoning skills to ensure a high degree of integrity in the decision-making processes.Flexibility and insightfulness and an ability to tailor established service delivery to meet the circumstances of each participant/worker.Ability to set clear expectations and engage in difficult conversations in a simple and empathetic manner.A range of experience working in relevant health or disability areas.Knowledge of the service system for people living with a disability.Knowledge of a person centred approach to service delivery.Experience and/or understanding of CTP/Workers Compensation legislation and guidelines.Experience working with people with brain and/or spinal cord injuries.CultureWe know our strength comes from the diversity of our people and would encourage people with different experiences and backgrounds to apply.
