Always.We are St Vincents Health Australia, Australias largest not-for-profit provider of health and aged care services.Founded by The Sisters of Charity in 1857, our +30,000 healthcare superheroes operate public and private hospitals, residential aged care, community and virtual care, and outreach programs.About the Role:Max-term full-time role delivering high quality, evidence-based mental health care and support to clients of the Wesley Mission SHS Team who live with mental illness and are experiencing or at risk of experiencing homelessness.Deliver comprehensive assessments, clinical intervention, care coordination, and referral on outreach alongside Wesley Mission staff.Collaborate with clients, multidisciplinary team, Wesley Mission, and stakeholders to address cultural, holistic health, and support needs using a strengths-based, harm reduction approach.Build strong partnerships and coordinate care with service providers and communities to strengthen client support networks.Assist clients in accessing mainstream and specialist healthcare, support services, and stable accommodation to improve health and wellbeing outcomes.Lead staff education, contribute to clinical reviews, and drive quality improvement across services.About You:Skilled, compassionate Social Worker, Occupational Therapist, or Registered Nurse with current AHPRA registration or eligibility for AASW membership, plus post-graduate qualifications.
