Due to an internal promotion, Infinite Care is currently seeking a dedicated Clinical Care Coordinator and Nurse Mentor. The successful candidate will be available to work the following shifts: Monday, Thursday, and Friday from 2:00 PM to 10:00 PM, and Saturday and Sunday from 9:00 AM to 5:00 PM.The RoleWe have an exciting and fast-paced opportunity to join our Wyoming NSW Clinical Care team as a Clinical Care Coordinator & Nurse Mentor. This full-time role reports to the Facility Manager, with support and guidance from the Care Manager, while supervising and supporting our passionate care team. The key purpose of this role is to ensure that all residents within our beautiful Chamberlain Gardens Wyoming NSW facility receive the highest quality of clinical care. The Care Coordinator is responsible for managing and resourcing the care needs within the facility, ensuring documentation and assessments are up-to-date and aligned with best-in-class clinical practices.Key ResponsibilitiesProvide clinical expertise and leadership to the clinical and care service teams across the facility.Assist the Care Manager in leading care teams to meet all clinical needs of residents, including monitoring and tracking clinical indicators to achieve optimal care outcomes.Manage all aspects of assessments, documentation, and submission of AN-ACC requirements within the facility.Support effective documentation and provide user support for policies, procedures, and IT systems within the facility.Coordinate assessment, care planning, and review processes.Support the admission of new residents, including care assessments and initial consultations with residents and families.Ensure accurate recording of information in accordance with organizational and legislative requirements (AN-ACC, resident records, staff time recording).Contribute to team rosters, ensuring an optimal skill mix for resident care.Manage and document clinical risks, and promote continuous improvement within the facility.You will offerInnate empathy for residents and a commitment to improving clinical care quality.Minimum 1 year experience as a Care Coordinator or similar leadership role.Availability to work the specified shifts: Monday, Thursday & Friday 2 PM to 10 PM, Saturday & Sunday 9 AM to 5 PM.Current unrestricted Registered Nurse Div 1 registration with AHPRA.Postgraduate qualifications in aged care are advantageous but not essential.Extensive knowledge of aged care clinical assessments, care plans, AN-ACC, accreditation compliance, and documentation.Ability to influence and develop clinical leadership skills within the team.High-level computing skills for documentation and reporting.Excellent communication and interpersonal skills, with a passion for leading clinical outcomes.Ability to manage multiple stakeholders with a balanced approach to emergency and stressful situations.Valid NDIS Workers Screening Clearance or ability to obtain one.If you would like to know more about us or this position, please contact our recruitment specialist, Victoria, at victoria.mulrin@infin8care.com.au.Benefits - Why work for us?Fitness PassportCareer growth opportunities within a strong mentoring cultureSpecialization opportunities in palliative, dementia, IPC, falls, and wound careFree access to in-house and online training programsSupportive, positive team environment with a strong caring work ethicSafety-first workplaces for residents, families, and staffWellbeing, fun, and engaging team activitiesFamily-friendly benefits like EAP and employee discountsFree professional development programsWhy Infinite Aged Care?We are an Australian aged care provider with over 30 years of experience.
