You will support participants to understand and implement their NDIS plans, build their capacity, and achieve their goals through meaningful connections with services and supports.Skills & ExperienceTo be successful in this role, you will have: A relevant tertiary qualification in Social Work, Human Services, or a related field. At least 2 years experience as a Support Coordinator, particularly in mental health and psychosocial disability. Solid understanding of the NDIS legislative framework and Support Coordination processes. The ability to work independently and collaboratively within a team environment.
