Tradewind Australia partners with service providers across the Aged Care sector to offer quality care to Victorian communities.An exciting opportunity exists for an experienced and forward-thinking Program Manager to lead the delivery of Home Care Package (HCP) and Commonwealth Home Support Program (CHSP) services in Bendigo.This is a critical leadership role focused on delivering client-directed care, supporting reablement goals and preparing the organisation for the significant changes ahead as Australia transitions to the new Support at Home (SAH) program.About the RoleAs Program Manager, you will be responsible for ensuring high-quality responsive care services that meet current aged care standards while also positioning the team and service model for a successful transition to SAH. You will lead a multidisciplinary team, champion person-centred care and drive outcomes aligned with client goals, wellbeing and independence.Key ResponsibilitiesManage and develop client services under HCP/CHSP with a consistent person-centred approach to careLead and support a multidisciplinary team to complete timely assessments and develop personalised care plansOversee referral pathways including to My Aged Care and relevant internal and external servicesEnsure clients receive accurate information about services, rights, responsibilities and available supportsPrepare the service and team for upcoming Support at Home program changes ensuring alignment with policy direction, funding reform and care model requirementsMonitor client outcomes, satisfaction and wellbeing through structured review processesChampion community participation, client feedback and program evaluation for continuous improvementMaintain effective workforce planning, rostering and staff engagement to meet care needs and maintain service qualityPromote a strong workplace culture of safety, inclusion, trust and professional developmentWhat You BringExtensive experience in managing HCP and/or CHSP programs in a community settingStrong working knowledge of current aged care reform particularly the Support at Home (SAH) program design, timelines and implications for service deliveryProven leadership and change management capabilitiesIn-depth understanding of person-centred care, reablement and wellness approachesDemonstrated ability to lead high-performing teams and collaborate across disciplinesSolid understanding of aged care compliance, Aged Care Quality Standards and clinical governance frameworksExcellent communication and stakeholder engagement skillsAlignment with a values-based care approach that prioritises dignity, choice and independence for older peopleWhat''s on OfferA meaningful leadership role with real community impactA collaborative and inclusive team environmentSalary packaging benefits to increase your take-home payOngoing professional development and trainingA unique opportunity to shape service delivery during a major aged care reformTradewind Australia is a specialist recruitment service for the Social, Community, and Aged Care sectors.
