The service is for clients with a mental health diagnosis who are eligible for a support package. The role of the Community Support Worker is to help the client access their local services and maximise their recovery success.Key ResponsibilitiesProvide dedicated support to clients with a mental health diagnosis, focusing on recovery and community living goalsCoach and encourage independence through daily routines and decision-makingAssist with cooking, cleaning, and home upkeep, empowering the client to take control of their daily livingEncourage community access and engagement in local activitiesSupport capacity building and self-management of lifestyle and wellbeingWork collaboratively with clinical and mental health support teamsFollow and contribute to individualised support plansThursday and Friday daytime availability REQUIREDFlexibility to travel up to 40 minutes from home, as required.
