From GPs and hospitals to aged care and pharmacies - we connect the dots so people get the right care, in the right place, at the right time.We support over 1.6 million people across our region, with a focus on aged care, mental health, Aboriginal and Torres Strait Islander health, digital health, and workforce development.What Youll DoJoining our Access & Referral team, youll be part of the critical first step in someones care journey - helping clients, GPs, and service providers navigate the mental health and alcohol and other drugs services with clarity, compassion and clinical confidence.In this fast-paced, purpose-led role, youll:Lead Clinical Triage: Reviewing referrals and matching clients with the right level of care across the stepped care model.Support and Guide: GPs, referrers, and providers to navigate options, manage risk, and make timely, effective referrals.Stay alert to complexity and risk: Taking a proactive approach to escalations and stepping in with sound clinical judgement.Troubleshoot the Tricky Stuff: Complex referrals, eligibility questions, or those cases that dont fit neatly in a box.Keep Things Moving: Ensure accurate, timely data entry and contribute to system improvements behind the scenes.Strengthen the System: Support implementation of the National Initial Assessment and Referral Framework.About YouYoure a clinician whos not afraid of complexity and thrives on bringing clarity.
