This wellbeing service delivers a primary health care service to the people of the Gold Coast & surrounding communities.About the Role:They are looking for a compassionate and experienced Registered Nurse to join their team on a full-time, fixed-term basis. This is a vital role supporting Aboriginal and Torres Strait Islander individuals and their families by delivering respectful, culturally appropriate, and high-quality care. This role will be based across our Miami, Bilinga and Coomera clinics.Position Duties:Conduct clinical assessments in the home for aged and elderly clients in line with evidence required for funded programs (e.g. RUDAS Assessments, PCOC assessments, health assessments).Provide in-home nursing care to patientsDeliver in-home care to clients including wound care, diabetic management, medication administration, stoma care and catheter and drain management.Escalate changes in patient condition to the team lead or program manager as requiredMaintain accurate clinical notesCollaborate with other care providers to ensure appropriate and timely client care.Provide backfill support in our Primary care clinics when required.Perform routine clinical dutiesSupport clients and their families through end-of-life discussions, grief, loss, and psychosocial needsSelection Criteria:Current registration with AHPRANursing qualificationPreference of at least 1 years experience in community nursingDemonstrated ability to deliver culturally safe care to Aboriginal and Torres Strait Islander clients and familiesExperience providing in-home palliative care, chronic disease management, and cancer care highly regardedCompetent in clinical tasks such as wound care, diabetic management, catheter care, stoma care and medication administrationFamiliarity with GP Practice Nurse responsibilities and ability to provide backfill support as neededStrong communication skills, including the ability to manage sensitive discussions around death, dying, and griefExperience with clinical information systems and data entry.Current CPR & First Aid CertificateAdditional Information:Satisfy a national criminal history check (less than 12 months old)Hold a current drivers'' licenseAboriginal and Torres Strait Islander people are strongly encouraged to apply.On Offer:The organisation values the contributions of all staff and is committed to providing an inclusive workplace where all staff feel accepted, included and welcomed.
