Base d in either Moe, Morwell or Traralgon (and required to travel to all these sites), youll report to our Team Leader, Gippsland.You will make a difference by:Building family capacity by using partnership, outcomes based, strengths perspective and client-centred approachesIdentifying where additional assistance and external services are requiredCollaborating with team members to deliver programs and support services, promoting positive and culturally respectful approaches to parenting, and child and family wellbeingProviding case management, delivering support to vulnerable and disadvantaged familiesCollecting and recording data and information for the purpose of accountability, evaluation, and reportingA place to feel valued:Youre a dedicated family support practitioner with a strong foundation in early childhood development.
