The Program is structured under three Specialty Medical units Cardiology, Cardiothoracic and Respiratory Medicine. Our Heart & Lung Transplant service provides a referral, assessment and transplantation service for adults with end stage heart and/or lung disease, as well as the National Paediatric Lung Transplant Service. A multidisciplinary team that includes surgeons, physicians, cardiologists, allied health and nursing staff manages each patient journey.The RoleThe role uses expert clinical knowledge to provide advanced patient triage and assessment that informs and contributes to the development of optimal care planning and treatment care pathways. The CNC uses this specialist knowledge to educate clinicians, patients, families and carers which in turn assists their understanding of their diagnosis and subsequent treatments.The Heart & Lung Transplant Coordinator After Hours, Clinical Nurse Consultant is the primary contact in the management and coordination of all heart & lung donor referrals and transplant coordination for all heart and lung recipients for the Alfred, after hours.Rostered shift from 16.00hrs- 22.30hrs, with on-call shift component from 22.30hrs through to 08.00hrs the next dayDays of work: Sunday on- call shift only (20.00hrs- 08.00hrs), Monday- Thursday evening shifts with on-call componentSkills & ExperienceCurrent Registered as a Registered Nurse with Nursing and Midwifery Board of Australia via AHPRA.Advanced clinical knowledge and skills in cardiothoracic; heart and /or lung transplantationPost graduate qualification in Acute or Critical Care preferred or working towards Masters qualificationExperience working in a senior clinical role, i.e. CNS/ ANM or aboveDemonstrated ability to work independently with minimal supervision and maintain energy and focus out of hoursDemonstrated ability to critically think, problem solve, multi-task, remain calm and resilient whilst under pressure and respond quickly to changing circumstancesStrong interpersonal and communication skillsAbility to work collaboratively with other services and departments across the hospital and communityAbility and willingness to develop othersCommitment to quality and safety activities / research process and application to practiceCurrent and satisfactory Police CheckCurrent and satisfactory Working with Children CheckStaff BenefitsSalary Packaging & Novated Leasing through Maxxia.Flexible Heath Insurance coverage through HCF Health InsuranceOn-site car & bike parking opportunities, Deducted Pre-Tax!!Fantastic onsite fitness facilities at The Alfred through ProSport Health and Fitness.Child Care Services at The Alfred managed by KU Childrens ServicesIf applicable, please specify specific requirements that you require in your cover letter or CV.For enquiries regarding this position, please contact Hilary Stedman at H.Stedman@alfred.org.auApplications close 11pm AEST on Thursday 31st July 2025.Alfred Health is an equal opportunity employer and is committed to attracting and retaining a diverse workforce that reflects the community we serve.
