As of January 2025, this system has replaced the Aged Care Assessment Service (ACAS) and Regional Assessment Service (RAS).Alfred Health Aged Care SAS provides comprehensive and home support assessments via the Victorian Dept of Health agreement. Comprehensive assessments are usually completed face to face in the hospital or community setting, to determine a clients eligibility for aged care types under the Act. Home support assessments are completed in a community setting for clients identified as having entry level needs that can be accommodated by Commonwealth Home Support Services.The geographical catchment for community assessments incorporates the Southern Metro Aged Care Planning Region.The RoleThis role will require you to travel to complete face- to- face assessments as well as telehealth and phone assessments as required.You will need to be adaptable to assessment demand and may be required to complete home support assessments.
