The Support Centre handles essential operational functions of service delivery management, finance management, scheduling, and compliance.The Home Care Package Coach plays a crucial role in delivering high-quality support in alignment with the VIVA Team Framework. This role promotes the autonomy of self-managed teams, enhancing team and client self-reliance, and fostering professional growth among Support Workers. Key responsibilities include mentoring and guiding support staff, ensuring quality service delivery, assisting with client onboarding and case reviews at the direction of the Aged Care Lead, and ensuring implementation of continuous improvement initiatives. The Home Care Package Coach works collaboratively within the Aged Care Service Delivery Team and other organizational teams to embed VIVA Mutual''s culture and values into daily practice. Coaches are also responsible for implementing organizational improvements at the service level. Position responsibilities will be reviewed from time to time in line with organizational need.Skills and Knowledge:Tertiary qualifications in social work, human services, or relevant experience.Ability to work collaboratively within self-managed teams, promoting autonomy and self-reliance.Demonstrated ability to effectively support and mentor peers in delivering high-quality, person-centred care.Strong understanding of and ability to apply Aged Care standards and principles in daily practice.Knowledge of relevant WHS regulations and quality management systems.Excellent communication and interpersonal skills, with the ability to build positive relationships with clients, families, team members and with a diverse range of stakeholders.Proficiency in conducting comprehensive client intake processes and developing individualized support plans.Strong problem-solving skills and the ability to handle complex situations with professionalism and empathy.Ability to build relationships and influence others positively within the organizationHigh level of organizational skills, with the ability to manage multiple tasks and prioritize effectively.Proficiency in using Microsoft Office and other relevant software systemsAdditional certifications or training in areas related to disability support, aged care, or community services.ExperienceMinimum 3 years of experience in a support work, coordination or equivalent role within the disability, aged care, or community services sector.Experience managing Home Care PackagesExperience in managing budgets within the scope of the funding sourceWorking closely with multidisciplinary team and approach to health careExperience using the My Aged Care PortalProven experience in mentoring and coaching support staff to enhance professional development and service delivery.Demonstrated ability to empower clients and support staff to achieve their goals and enhance their capacity for self-management.Experience in maintaining accurate client records and ensuring compliance with relevant legislative and regulatory requirements.Required Compliance Items:National Criminal History Check,NDIS Worker Screening ClearanceWorking With Children CheckNDIS Worker Orientation ModulesDrivers License.Own VehicleWhy work with VIVA?VIVA Mutual is a not-for-profit organisation that delivers local, personalised health and wellbeing support services through a connected network of dedicated support workers, and health care professionals.VIVA is about living.Engaged Employees | Valued ClientsWe recognise staff as our strongest asset.
