If you can dream it, we can support you in achieving it.Job DescriptionThe Support Coordinator plays an integral role for NDIS participants in interpreting their NDIS plan, assisting them to establish the supports in their plan, and developing their capacity over time to coordinate and manage their own supports.Primary Role:Assist NDIS participants to:Implement their plan and increase their capacity to manage/direct their own supportHave greater opportunities to explore and connect with their community and alternative support optionsStrengthen their informal support networksMaximize the value for money they receive from their supportsAccess and use the NDIS Participant Portal (my place) Effectively communicate and maintain relationships with the NDIA. Maintain a company KPI of bringing in 1 client per week to Point Care for Support Coordination (thereby increasing their caseload).Accountability:Action referrals in a timely mannerSupport the NDIS participant to:Assess a number of mainstream, community, informal, and provider optionsSelect preferred options or providersNegotiate services to be provided and their pricesDevelop service agreements and create service bookings with preferred providersArrange for any assessments required to determine the nature and type of funding requiredDetermine the budget for each support type and advise relevant plan managers of the breakdown of fundsLiaise with plan managers to establish appropriate claim categories and attribute the correct amount of fundsLink to mainstream or community services (housing, education, transport, health)Strengthen and enhance participants'' capacity to coordinate, self-direct, and participate in the community, including providing assistance to resolve problems, understand responsibilities, and change or end support agreementsBe the key contact for service issues, complaints, major changes, and plan reviewsAssist participants to prepare for their plan review by assessing goal achievement and value for moneyIdentify solutions to problems experienced in implementing the planConsider new goals and actions for achieving goals, including exploring housing options and life transition planningMaintain confidentiality of customers, families, carers, and the organizationNetwork internally and externally to assist individuals in achieving personal outcomesManage conflicts of interest according to NDIAs Terms of BusinessProvide NDIA with reports on goals, outcomes, and success indicators as per reporting scheduleContinuously improve Point Care''s reputation through practice, business development, and innovationRequirementsA commitment to the Code of Conduct and Point Care policies and proceduresExcellent interpersonal and active listening skillsExcellent written and verbal communication skillsAbility to adapt communication style to meet people''s needsConflict resolution skillsHigh numeracy and literacy levelsStrong administrative skills including time management and task coordinationProficiency with systems and technologyAbility to work independentlyAbility to build and maintain community connections and relationships with service providers, individuals, and families/carersUnderstanding of the NDIS Price Guide, legislation, rules, and the role of mainstream services, including child safety and advocacyWorking as an NDIS Support Coordinator offers the opportunity to make a positive impact on people''s lives, stay current with industry developments, work in a supportive team environment, and enjoy career growth and job security in a growing government-funded program. #J-18808-Ljbffr
