Youll be supported by a team that values you and nurtures your professional growth.About The RoleWe''re looking for a Commonwealth Home Support Programme (CHSP) Coordinator to join our team.As a CHSP Coordinator, youll be responsible for managing and coordinating support services including Flexible Respite, Domestic Assistance, and Home & Garden Maintenance. Youll play a vital role in ensuring services are tailored to meet individual client needs, goals, and preferences.Responsibilities include:Conducting regular client reviews and home visits to assess care needsIdentifying and addressing risks, and following up on incidents and feedbackLiaising with families, support networks, and health professionalsMaintaining accurate and up-to-date care plans, agreements, and documentationManaging complaints and feedback in a timely, professional mannerParticipating in on-call duties every sixth week (with RDO in lieu)Supporting other tasks as directed by managementAbout YouYoure a compassionate and organised professional who brings both heart and structure to your work.
