A transitioning economy is leading to an influx of new residents, businesses, and employment opportunities that is seeing Maribyrnong fast becoming a sought after residential, recreation and social destination in Melbourne''s inner-west.The OpportunityThe Active Ageing Unit has an exciting opportunity for a Social Support Program Lead to join the team. As our Social Support Program Lead, you will be responsible for the planning, delivery and evaluation of Social Support programs that align with the Commonwealth Home Support Programme (CHSP) and Home and Community Care for Younger People (HACCPYP) funded programs, in addition to the leadership of staff that support the program''s delivery. You will also implement appropriate policies, procedures and processes required to ensure the appropriate management and delivery of the program with respect to procurement, planning, financial purchasing, and staff management.Key ResponsibilitiesManage all aspects of the Social Support program including the implementation of a co-design approach to program planning, the management of all financial requirements associated with the program, undertaking site visits and development risk assessments, and the management of transportation and booking needs for the program.Manage all referrals, Care Plans and regular communication with current and prospective Social Support clients, ensuring all client information is maintained and updated in the appropriate systems.Provide support and guidance to support staff in the delivery and management of high quality Social Support Programs.
