We are currently looking for a permanent community based Clinical or Non-Clinical Care Partner to join our Sunshine Coast team (Monday-Friday 8:30am-4:30pm). Position location in Gympie.Join a growing national network where you can make a difference and practice your values in a positive, friendly and flexible administrative work environment.Duties and responsibilities may include:Undertaking Assessments of participants in their homesEnsuring follow up to reduce risks, refer allied health, order equipment etc and evidencingOngoing case management supporting participants in clinical and holistic needsMonitoring and communicating on the participants wellbeing and reporting on progress of participants including, but not limited, to care assessments, care plans and health care plansDocumenting participants goals, progress and outcomes (journey) using person centred, wellness, reablement and ablement supporting strategies (progress goals and outcomes)Undertake administrative duties including data entry, processing invoices, following up participant enquiries, ensuring documenting all interactions to ensure compliance etc.Ensuring evidence is gathered and uploaded including quotes, confirmation of delivery of products and services etc.Documenting all evidence requirements in line with Compliance, ensuring 100% ComplianceOverseeing and managing participants Budgets (Govt Funding)Working with or under the guidance of medical practitioners and Allied Health professionalsEnsuring that all services are provided in a caring and respectful way in accordance with Just Better Cares policy and procedures, legislative requirements and program specific guidelines (i.e. Home Care Program Support At Home Program)Providing training, coaching and guidance to Community Support Professionals, Enrolled Nurses and Support Staff/Care Partner colleagues as requiredConducting skills assessments of Community Support Professionals performing complex care services as requiredAssist in participant service schedules including service planning, confirmation and short notice placements or cancellationsLiaison with Aged Care Assessment Team (ACAT), My Aged Care (MAC) Portal, PRODA and other regulatory bodiesDemonstrating a commitment to best practice and evidence of continued professional developmentParticipating in staff meetings and attending training opportunities when requiredOccasional Wound Care (Clinical Care Partner Only)What you will need:Advanced computer skills (CRM) (MUST)Advanced administration skills and organisational skills (MUST)Ability to multi-task and work in a fast paced working environment (MUST)Strong skills in prioritising and problem solving (MUST)Ability to work in a wellness model reablement and enablementDemonstrated strengths in team work and working autonomouslyPrevious experience in KPIs and working to deadlinesCurrent CPR / First Aid CertificateCurrent and satisfactory Criminal Record CheckCurrent and satisfactory NDIS Screening Card and Working With Children CheckCurrent Australian Drivers LicencePrevious experience in the home care package, community care or health/aged care sectorUnderstanding of Home Care Package program Guidelines and Compliance or previous community nursing experienceMinimum Bachelors in Nursing - Current AHPRAEnrolled Nurse - Current AHPRAWhat we offer:Use of company vehicle availableLaptop and mobile phoneTraining & professional development opportunities (including free access to online learning tools & programs)Access to our free Employee Assistance ProgramStaff recognition programsOngoing support from friendly, local office based teamFlexible working environmentNext steps:If you would like to join our growing team and are enthusiastic about administration work and attention to detail, and want become part of our friendly, professional network please apply today.
