Enjoy great rewards and benefits.Permanent Full Time leadership position; Mondays to Fridays: 0830h - 1636hSalary Packaging benefits up to $15,900 annually to maximise your take home payEmployee well-being program with great discountsAbout The RoleIn this role you will deliver wellness and lifestyle services and activities that empowers and support residents to engage in meaningful activities that improve quality of life and align with SCCs Better for life model.Key responsibilities of this role include but are not limited to;facilitating and coordinating direct reports including staff, volunteers and students to provide person centred safe and effective wellness and lifestyle programssupporting and enabling residents to maximise their independence, improving and maintaining their health, wellness and quality of lifeusing best practice and evidence based tools to plan and deliver high quality lifestyle and exercise services with the emphasis on person centred care and healthy ageing through goal setting, emotional support and health literacyenabling participation in both community and facility activities of relevance to each residentworking as part of a multi-disciplinary Early Intervention team to identify and develop Recovery Pathways for residents at risk of decline and frailtyundertaking audits to ensure all residential Lifestyle programs operate in accordance with Southern Cross Care procedureimplementing organisational projects to improve client outcomes as directed by the central Wellness and Lifestyle teamFor further details regarding the scope of the role, the attached position description.About YouWe are looking for people who will be the difference in the lives of our residents and clients.The successful candidate will have the following skills and experience:minimum Diploma level qualification in health promotion, social work, dementia care, health coaching, healthy ageing or leisure and lifestylesound knowledge of the Aged Care Standards particularly as they relate to accreditationexceptional interpersonal skills including the ability to coordinate and motivate a team of staff, volunteers and studentsdemonstrated experience with auditing and reporting, documenting person centred assessments as well as planning, developing, implementing and evaluating wellness and lifestyle programs for older adultssound best practice skills for early identification of social withdrawal and decline and case management to improve resident outcomesgood understanding of the requirements to support residents living with dementiaexperience in undertaking regular consultation with clients and their familiesexceptional communication, organisational and relationship building skillssound problem solving skills particularly when assessing balancing supporting residents desire for independence and self determination to make their own choices against potential risks and duty of careproficient computer skills and the ability to use Google Suite and in-house databasesAbout UsSouthern Cross Care is one of Australias leading charitable aged care and retirement living service providers.
