This role enhances access to healthcare for "Older Persons''" by addressing service gaps within dynamic Models of Care (MoC). Improving the health of older persons by providing timely access to specialist aged healthcare, preventing deterioration, reducing hospital presentations and admissions, while collaborating with other healthcare team members.The Transitional Nurse Practitioner is appointed for a provisional period of up to 3 years to work towards endorsement as a Nurse Practitioner.About Northern NSW Local Health District (NNSWLHD)We are a healthcare community, caring for the people of the Northern Rivers.
