Families receive intensive in-home and community-based support to address child safety concerns and ensure children and families are strong and connected. We provide collaborative case management using a multi-disciplinary wrap around support approach.As a Case Manager, you will play a vital role in supporting families through a collaborative, therapeutic case management approach. Working within a multidisciplinary team, youll partner closely with families, the Department of Human Services (DHS), the Department for Child Protection (DCP), and other professionals to promote child safety and family wellbeing.You will serve as the key worker for families, coordinating and integrating supports across services. Your responsibilities will include delivering practical in-home support, strengthening parenting skills, fostering connections to community and culture, and advocating for families to address safety and risk factors.This is a meaningful opportunity to make a real difference in the lives of children and families, using a relationship-based, culturally responsive approach.Key responsibilitiesAssessment of parental strengths and areas of need (including child protection risk and protective factors) and lead the development of a case planUtilise and support the use of the evidence-based assessment tools - North Carolina Family Assessment Scale (NCFAS), and other tools.
