You will work alongside the Residential Manager, fostering a collaborative approach to quality service delivery that meets the needs of all stakeholders, particularly our elders.Care Management Responsibilities IncludeEnsuring resident care and services are consistent with the Aged Care Quality Standards, using a multidisciplinary approach to address gapsFamiliar with the new Aged Care Act and strengthened Aged Care Quality Standards, with an understanding of key reforms in consumer rights, provider responsibilities and quality care deliveryLeading, mentoring and educating staff to ensure quality services are delivered to residentsOverseeing clinical governance and implementing clinical care needs and systemsLeading and contributing to quality improvement, accreditation and continuous improvement initiativesMonitoring the effectiveness of equipment, supplies and practices to ensure compliance with MiCare policies and proceduresManaging incidents, complaints, risks, and issues effectively and transparentlyEngaging in innovation, research and partnership opportunities to ensure MiCares services remain contemporary and leading-edgeSupporting the development and review of organisational policies and proceduresProviding regular reporting on key performance indicators (KPIs) and contributing to financial sustainability through a strong understanding of the Australian National Aged Care Classification (AN-ACC) funding model, including accurate resident classification, care minute targets, and alignment of staffing resources, as well as effective consumable cost control.Supporting residents social inclusion, spiritual and cultural needs while fostering relationships with families, staff, volunteers, and external service providersKey Requirements (in Addition To The Position Description Attached)Minimum 3 years experience as a Care Manager in the aged care sectorProven clinical and management expertise in a Memory Support Unit settingStrong leadership, communication and team collaboration skillsFlexibility to transition to a full-time positionQualificationsTertiary qualification in Nursing is essential.
