This role plays a crucial part in supporting the operational and strategic management of the residential service, including all clinical, customer and quality outcomes related to care delivery.The Care Manager ensures care reflects individual preferences and culturally appropriate practices while assisting clients to understand and access the level of support they require. You will work alongside the Residential Manager, fostering a collaborative approach to quality service delivery that meets the needs of all stakeholders, particularly our elders.Care Management Responsibilities IncludeEnsuring resident care and services are consistent with the Aged Care Quality Standards, using a multidisciplinary approach to address gapsFamiliar with the new Aged Care Act and strengthened Aged Care Quality Standards, with an understanding of key reforms in consumer rights, provider responsibilities and quality care deliveryLeading, mentoring and educating staff to ensure quality services are delivered to residentsOverseeing clinical governance and implementing clinical care needs and systemsLeading and contributing to quality improvement, accreditation and continuous improvement initiativesMonitoring the effectiveness of equipment, supplies and practices to ensure compliance with MiCare policies and proceduresManaging incidents, complaints, risks, and issues effectively and transparentlyEngaging in innovation, research and partnership opportunities to ensure MiCares services remain contemporary and leading-edgeSupporting the development and review of organisational policies and proceduresProviding regular reporting on key performance indicators (KPIs) and contributing to financial sustainability through a strong understanding of the Australian National Aged Care Classification (AN-ACC) funding model, including accurate resident classification, care minute targets, and alignment of staffing resources, as well as effective consumable cost control.Supporting residents social inclusion, spiritual and cultural needs while fostering relationships with families, staff, volunteers, and external service providersKey Requirements (in Addition To The Position Description Attached)Minimum 3 years experience as a Care Manager in the aged care sectorProven clinical and management expertise in a Memory Support Unit settingStrong leadership, communication and team collaboration skillsFlexibility to transition to a full-time positionQualificationsTertiary qualification in Nursing is essential. Qualifications in Social Work or Allied Health at a Bachelor level or higher are also considered.Current AHPRA registration highly regardedSkills & ExperienceExtensive experience in aged care management, with a broad clinical background and expertise in residential careDemonstrated leadership and mentoring skills to develop, support and motivate staffComprehensive understanding of the Aged Care Quality Standards and contemporary clinical care practicesStrong knowledge of aged care legislation, clinical governance, and quality and risk management frameworksProven ability to oversee and implement clinical care systems, including continuous improvement and accreditation processesExcellent communication skills, including the ability to liaise effectively with residents, families, staff, and external stakeholdersSound judgement and ability to manage incidents, complaints, and issues proactivelyIntermediate to advanced computer skills across a broad range of software applicationsDemonstrated ability to drive change and innovation in aged care settingsAbility to meet compliance requirements of the role, including:Current First Aid and CPR certificateValid drivers licenceEvidence of current flu and COVID-19 vaccinationsWhy MiCare?Competitive salary + Super + the ability to salary package up to $15,900 + meals and entertainmentEmployee wellness portal access great discountsFamily-friendly, work-life balance workplaceHybrid client-facing roleOpportunity to make a real difference in the lives of elders by providing exemplary, person-centred careAbout UsMiCare is a non-profit providing services to people from a range of diverse backgrounds in the areas of aged care, home care, retirement living and migrant services.
