Job DescriptionJob DescriptionKey Responsibilities:Lead the implementation, management, and continuous improvement of Balanced Wellbeing Dementia Care Program.Supervise and support dementia care team staff.Ensure full compliance with CMS GUIDE Model standards, including documentation, reporting, and quality benchmarks.Conduct assessments and develop individualized care plans tailored to each members needs.Monitor member progress and adjust care plans as needed to achieve optimal outcomes.Speaking with patients POA''s, facility staff and patients dailyProvide education, resources, and emotional support to caregivers to help reduce caregiver burden and promote overall well-being.Develop and maintain community partnerships to facilitate access to respite care, transportation, nutrition services, and other critical supportsUsing psychiatric scales for assessments.Scheduling unstable visits for the patients as neededMonitors adherence to care plansServe as a subject matter expert on dementia care, representing Balanced Wellbeing in GUIDE Model meetings.Participate in ongoing professional development to stay current with best practices in dementia care and social work.Requirements:Must have current Florida LPN license.Strong knowledge of Chronic Care Management/ Behavioral Health IntegrationValid Drivers License (we do run a background check)Excellent communication and interpersonal skillsAbility to prioritize tasks and manage multiple responsibilitiesProficient in electronic medical record systems and other relevant software applicationsStrong computer skillsExcellent communication skills #J-18808-Ljbffr
