The Youth Pathways and Reconnection Program is a case management service primarily focused on supporting young people who are newly homeless or at risk of homelessness. This program provides an early intervention response to prevent young people from entering the homelessness service system and to assist them to remain connected to family and their community. It has been recognised that youth homelessness extends into adulthood. Therefore, this service places significance upon the promotion and support of reconciliation with families with the aim of the young person remaining at or returning home.The position will also be required to provide case management support for young people presenting or residing at the Wodonga Youth Refuge who are residing there in a voluntary capacity.
