Your responsibilities will include:Efficiently and effectively dispensing and distributing medication.Conducting medication history interviews, performing medication reconciliation, and preparing discharge summaries during transitions of care.Offering medication counselling and education to patients.Collaborating with multidisciplinary teams to provide advice on patient care.Contributing to training and educational sessions for pharmacy staff, nursing staff, medical personnel, and allied health professionals.Assisting in overseeing imprest services and ensuring the continuity of medication supply.Considering the financial implications of medication use both within the hospital and for patients obtaining medicines in the community.About YouWe encourage all pharmacists to apply, whether you''re an experienced Hospital Pharmacist or a Community Pharmacist eager for your first experience in hospital pharmacy.To be considered for the role, you will need to possess the following:Current Pharmacist registration with AHPRAPrevious pharmacy experience within Australia or New ZealandStrong ability to manage your workload independently, paired with excellent communication skills to collaborate effectively with a diverse team of healthcare professionalsA genuine passion for advancing your clinical skills and knowledgeAbout UsIcon Group provides a complete suite of services to support the operation of more than 75 pharmacies across Australia.
