Home Care Package Care Partner/ Clinical Care PartnerJob No:JBC20242280Office: Sunshine CoastEmployment Status:Full-TimeRecruiting for a Full Time Home Care Package Care PartnerWe are currently looking for an experienced Home Care Package Care Partner, or registered nurse or enrolled nurse to join our Sunshine Coast team Monday to Friday, 8.30am 4.30pm. For over 20 years, we have been supporting older people, and people living with disability, to live independently in their own homes and stay connected to their local community.Our friendly, compassionate and experienced team makes this possible.Duties and responsibilities may include:Undertaking Assessments of participants in their homesEnsuring follow up to reduce risks, refer allied health, order equipment etc and evidencingOngoing case management supporting participants in clinical and holistic needsMonitoring and communicating on the participants wellbeing and reporting on progress of participants including, but not limited, to care assessments, care plans and health care plansDocumenting participants goals, progress and outcomes (journey) using person centred, wellness, reablement and ablement supporting strategies (progress goals and outcomes)Undertake administrative duties including data entry, processing invoices, following up participant enquiries, ensuring documenting all interactions to ensure program Guidelines are followed and ongoing complianceis achieved etc.Ensuring evidence is gathered and uploaded including quotes, confirmation of delivery of products and services etc.Liaise with relevant community based agencies and service providers to ensure an integrated and cohesive approach to service delivery and to ensure standards are met in accordance with program objectivesAssist in participant service schedules including service planning, confirmation and short notice placements or cancellationsWork with participants with their budgets and monitor participants budgets in accordance with program guidelines.Communication with key stakeholders, internally and externally.Community Engagement:- engagement with key community stakeholders, promoting Just Better CareDocumenting all evidence requirements in line with Compliance, ensuring 100% ComplianceOverseeing and managing participants Budgets (Govt Funding)Working with or under the guidance of medical practitioners and Allied Health professionalsEnsuring that all services are provided in a caring and respectful way in accordance with Just Better Cares policy and procedures, legislative requirements and program specific guidelines (i.e. Home Care Program Support At Home Program)Providing training, coaching and guidance to Community Support Professionals, Enrolled Nurses and Support Staff/Care Partner colleagues as requiredConducting skills assessments of Community Support Professionals performing complex care services as requiredDemonstrating a commitment to best practice and evidence of continued professional developmentParticipating in staff meetings and attending training opportunities when requiredOccasional Wound Care (Clinical Care Partner Only)Ensuring that all services are provided in a caring and respectful way in accordance with Just Better Cares policy and procedures, legislative requirements and program specific guidelinesLiaison with Aged Care Assessment Team (ACAT), My Aged Care (MAC) Portal, PRODA and other regulatory bodiesAssist participants to access other alternative services where the services are no longer meeting their needsIdentify and participate in personal professional development.Apply and uphold principles of equity and anti-discrimination in the workplace and adhere to organisational and legislative Health, Safety and Environment requirementsAuditing of participant files regularly to ensure complianceConducting skills assessments of Community Support Professionals who perform complex care services as requiredDemonstrating a commitment to best practice and evidence of continued professional developmentParticipating in staff meetings and attending training opportunities when requiredAny other duties as required to meet the ongoing needs of the organisationWhat you will need:Advanced computer skills (CRM) (MUST)Advanced administration skills and organisational skills (MUST)Ability to multi-task and work in a fast paced working environment (MUST)Strong skills in prioritising and problem solving (MUST)Ability to work in a wellness model reablement and enablementDemonstrated strengths in team work and working autonomouslyPrevious experience in KPIs and working to deadlinesCurrent CPR / First Aid CertificateCurrent and satisfactory Criminal Record CheckCurrent and satisfactory NDIS Screening Card and Working With Children CheckCurrent Australian Drivers LicencePrevious experience in the home care package, community care or health/aged care sectorUnderstanding of Home Care Package program Guidelines and Compliance or previous community nursing experienceMinimum Bachelors in Nursing- Current AHPRAEnrolled Nurse- Current AHPRANext steps:If you would like to join our growing team and become part of our friendly, professional network, and you are enthusiastic about administration work and following attention to detail, please apply today.
