This role is responsible for conducting AN-ACC assessments, developing and maintaining individualised care plans, and championing resident-centred care practices. The successful candidate will work in close partnership with the care team, residents, families, and the Facility Manager to drive high-quality outcomes through evidence-based assessments and thoughtful care strategies.Key ResponsibilitiesConduct AN-ACC Assessments in collaboration with care staff and leadership.Design and implement best-practice care plans that reflect individual needs and values.Undertake Resident of the Day (ROD) reviews alongside the Facility Manager.Ensure assessments are completed within 72 hours of admission and updated as required.Consult with residents, families, Div 1 nurses, and external providers to guide care planning.Lead continuous improvement initiatives through audits and feedback loops.Support carers and deliver emotional, social and clinical support to residents.Manage specialised care areas including mobility, nutrition, wound care, pressure care, continence etc.Maintain thorough documentation and communication across care teams and families.Complete HELF Checks and coordinate required training with PAL and P&C for new staff.Essential skills and attributes (selection criteria):Registered Division 1 Nurse with current National Practicing RegistrationExcellent problem-solving and care planning capabilitiesStrong working knowledge of the needs of the elderly person residing within a residential care facility.Exceptional problem solving, assessment and care planning skillsExcellent AN-ACC operational knowledgeSelf-motivated, proficient with computerised documentation systems and recordsStrong interpersonal skills with the ability to build and maintain working relationships within a multi-disciplinary teamBenefits & Culture We value our team and the wonderful work they do.
