Wellways works in partnership with the community and other organisations to deliver this vital service in the Murrumbidgee region.About the opportunityWe are seeking a compassionate and motivated Support Coordinator to join this newly created team. In this role, you will walk alongside individuals for up to three months following a suicidal crisis or attempt, offering tailored support to help them reconnect with life and community.You will work under the guidance of the Team Leader and in collaboration with internal and external stakeholders to support the ongoing delivery of this important service.Responsibilities will include:Working collaboratively with participants with a recovery or strengths-based approach to formulate, implement and review support plans, needs assessments and safety plans and to monitor progress through completion and review of outcome measuresParticipation in intake and assessment processes with referral partnersAssisting participants to connect with relevant health, clinical and community-based support servicesRelated administration such as (but not limited to) maintenance of client information records and activities, monitoring and evaluation activities and effective management and sharing of information with other service providers where requiredTo be successful in this role, you will need:Relevant qualifications or commensurate experience working in a support role for people with a mental health challengeExperience in working with children/young people is highly regardedExperience in working with people bereaved though suicide or experience in dealing with severe personal traumaAn understanding of the biopsychosocial model of mental health and how social inclusion principles are applied to service deliverStrong communication and interpersonal skillsRight to work within AustraliaCurrent valid Drivers License and the ability to undertake some travel and be away from home overnightNDIS Worker Orientation Module Certificate (free online course)NDIS Worker Screening CheckSatisfactory National Police Records CheckWorking with Children''s Check (Employee/Paid)Why us?Wellways are a team of approximately 2000 staff and 150 volunteers across 93 sites providing a range of programs, services and supports to people right across Queensland, New South Wales, Australian Capital Territory, Victoria and Tasmania.
