This is a hybrid working role, and regular travel across Grafton, Lismore, Tweed Heads and Coffs Harbour is a requirement of this position.Key ResponsibilitiesDevelop and implement high quality therapeutic case plans for Children and Young People in LWB''s careDevelop leaving care plans for each young person over 16 to support their transition to independent livingAttend shifts within the intensive therapeutic care homes, and work closely with the teamDrive the implementation of Leaving care Plans / P4I Plans / Cultural Support Plans and achievement of the case plan goalsActively engage children, young people, their families and link them in with the local community servicesParticipate in an on-call roster as requiredEnsure compliance with all internal policy and procedure, statutory and contractual requirements in relation to management of service deliverySkills & ExperienceRelevant bachelors degree, or a relevant diploma and be working towards a degree.
