We are seeking a Community Worker who will work closely with individuals, families, and community groups to assess needs, create tailored support plans, and connect people with the resources and opportunities that will improve their quality of life. Key Responsibilities Assess clients needs and develop individualised support programs. Interview clients to understand their challenges and strengths. Plan, deliver, and evaluate community-based programs, workshops, and activities. Monitor client progress and prepare progress reports. Refer clients to external agencies for specialised assistance when required. Assess community needs and identify available resources in areas such as health, housing, employment, and training. Build partnerships with community groups, welfare agencies, government bodies, and local businesses to promote awareness of services. Support families and provide education and care for children, young people, and adults in community-based programs. Assist young people to address social, emotional, and financial challenges. Prepare reports, funding submissions, and documentation to support service delivery. Skills & Experience Certificate IV or Diploma in Community Services, Social Work, or related field (or equivalent experience). Demonstrated experience working in community services, social welfare, or a related sector. Strong interpersonal and communication skills to work effectively with people from diverse backgrounds. Knowledge of local community resources, referral pathways, and relevant legislation. Organisational skills to manage multiple clients and programs effectively. Current Drivers Licence and ability to travel within the community. Current Working With Children Check and NDIS Worker Screening Clearance (or willingness to obtain).
