Were looking for a caring and capable Community Worker to join our team and make a real difference in peoples lives. What youll do Meet with clients to understand their needs, strengths, and goals. Create and deliver personalised support programs, including skill-building, social inclusion activities, and daily living assistance. Keep track of progress, write clear reports, and update plans as clients needs change. Link clients with other services and resources in the community. Work with families, community groups, and other agencies to promote access to health, housing, education, training, and employment opportunities. Provide respectful care and education for people with disability in home, group, and community settings. Support young people and families to address social, emotional, and financial challenges. Work closely with families, carers, case managers, allied health professionals, and other team members to coordinate support. Prepare funding requests and maintain accurate records for service delivery. What were looking for Certificate III in Community Services, Disability, or equivalent. At least 2 years experience in the NDIS setting. Proficiency in assessing needs, planning and delivering support programs, case interviewing, and reporting progress. Solid interpersonal and communication skills to effectively liaise with clients, agencies, and community partners. Current First Aid certificate, valid drivers licence, and police clearance. Ability to work well with people from all backgrounds, showing empathy, patience, and respect. Confidence in building relationships with clients, families, and service providers.
