We aim to do this by working with our rural communities to provide responsive and innovative health and aged care services to enable our communities to remain in their local area.About the OpportunityBased in Scottsdale, the Social Club Coordinator coordinates day to day operations of the social club program in Scottsdale and Lunch with Friends program based in Swansea, to ensure optimal delivery of support at all times. Supporting a small team of social club and lunch with friends staff, the Social Club Coordinator is responsible for the day-to-day coordination of both programs.Key ResponsibilitiesManage and coordinate the day to day running of the Social Club and Lunch with Friends Programs, in consultation with the Director of Aged and Community Care.Plan and develop Social Club activities alongside staff, volunteers and consumersHold or the ability to obtain a public passenger vehicle licence to provide safe bus transport for consumers.Complete a visual safety check of the bus before use and maintain the bus in a safe clean manner.Maintain social club consumer profiles, documentation, records and statistics as required by May Shaw Health CentreCommunicate and interact with the multidisciplinary team at May Shaw to meet consumer needs.Participate in health promotion activities targeting the Day Centre consumers in conjunction with other team members.Actively participate in quality assurance programs and accreditation of the social club and lunch with friends.Personal accountability for the standard of care delivered in accordance with this organisations care practice, policies and procedures, Care Standards, and Duty of Care.Understand the Charter of Care Recipients Rights and Responsibilities in Home Care, and the organisations requirements on Consumer Abuse and Mandatory Reporting, consumer family and organisational confidentiality.Support and mentor new staff as necessary and in a team environment, in consultation with the Director of Aged and Community Care.Ensure WH&S guidelines are followed and be responsible to report incident/near miss, adverse events (consumer related incident) and hazard reports in a timely manner.Adhere to infection control measures.Undertake other duties relevant to skills and experience, as assigned by the Director of Aged and Community Care.Comply with all State and Federal Laws, organisation policies and procedures.Participate in and contribute to quality improvement programs and other organisational activities to meet accreditation standards and promote quality improvement of services provided.Participate in annual mandatory training and ongoing personal and professional development.Attend and participate in staff meetings.
