Join to apply for the Social Worker Integrated Care role at Far West Local Health DistrictLocation: Broken Hill, New South WalesOverviewThis permanent full-time position provides a high-quality Social Work service to patients, clients, consumers and their family/carers within the Integrated Care Team.ResponsibilitiesProvide a high quality Social Work service to patients, clients, consumers and their family/carersEnsure the provision of optimal health outcomes consistent with FWLHD policies, procedures, standards, relevant legislation, and the NSW Health Code of ConductWork as part of the Enhanced Care Team within the Integrated Care TeamQualifications and ExperienceA bachelor degree in Social Work which provides eligibility for membership of the Australian Association of Social Workers and a minimum of 3 years post graduate experience in a relevant field.
