Working closely with clients, families, and support staff, you''ll ensure services are delivered with compassion, professionalism, and efficiency.Your day-to-day will include:Managing incoming referrals and conducting client assessmentsDeveloping individualised care plans tailored to client goals and needsCoordinating staff schedules and matching carers with clientsMonitoring service delivery to maintain quality and client satisfactionMaintaining accurate client records and complying with relevant policies and proceduresResponding promptly to incidents, feedback, or concernsOn-call roster - allowances and call out levy - additional $What Were Looking ForWere after someone who combines warmth and empathy with sharp organisational and communication skills.
