We are seeking a Healthcare Transformation Consultant to join our team. In this role, you will play a crucial part in improving patient outcomes, addressing health equity, and shaping the future of healthcare delivery in a rapidly growing sector.Responsibilities:Assist physician offices in achieving Patient-Centered Medical Home model recognition and managing provider metricsOptimize workflow processes and support clients in meeting project timelines and quality expectationsEngage in business development activities to enhance client base and generate revenueDevelop work plans, present project results, and educate healthcare providers/practicesSupport client interactions with public health agencies and keep abreast of regulatory changes and trendsCoordinate project activities and resources, ensuring project targets and completion dates are metPrepare and update performance reports for supervisor reviewStay current with quality improvement methodologies, EHR adoption, and clinical operationsDemonstrate strong project management skills, including organization, planning, prioritization, and time managementQualifications:Prior experience with Patient-Centered Medical Home and practice transformationKnowledge of healthcare revenue cycle, Alternative Payment Models, and quality improvement methodologiesFamiliarity with EHR implementation, MIPS/Meaningful Use, and HEDISStrong interpersonal, communication, and public speaking skillsAbility to work well under pressure, meet deadlines, and function effectively in a high-intensity environmentProactive, self-motivated, and capable of working autonomouslyProficiency in using Salesforce CRM or similar project management toolsBachelor''s Degree in Health Informatics, Health Services Administration, Health Sciences, or related field preferred; relevant healthcare industry experience also consideredIf you are dedicated to making a positive impact in healthcare and possess the necessary skills and experience, we invite you to apply for the Healthcare Transformation Consultant position.Salary Considerations: $50,000 - $70,000 (based on experience) #J-18808-Ljbffr
