Collaborates with patient, family, physician, and interdisciplinary team to assure care is provided from acute care through the continuum. Includes taking a leadership role with the interdisciplinary team to achieve actions and processes designed to ensure optimal clinical outcomes, coordination, and continuity of health care as patients transfer between different levels of care and promotes effective utilization of healthcare resources through monitoring the patient''s hospitalization.
