OverviewJob Title: Clinic NurseReq: 2025-1027Location: Newcastle Primary Care, Newcastle, WADepartment: NewcastleShift: DaysType: Full TimeFTE: 1Hours: Monday - Friday; Hours may vary to meet departmental and patient needs.Reports To: Clinic ManagerSalary Range: Min $94,894 - Max $142,341 annual DOECategory: ProfessionalPosition: Clinic Nurse in a outpatient clinic setting, working with clinical care teams to coordinate care, respond to calls and inquiries from patients and families, and apply the nursing process per scope and standards of practice.ResponsibilitiesUse Nursing Process to assess, plan, implement, and evaluate patient needs.Perform and delegate tasks to facilitate patient care.Serve as a clinical resource and provide guidance to call center staff for clinical questions.Improve or enhance triage and patient advice services within the clinic.Assess and respond to urgent or emergent situations; triage and respond to incoming patient calls and MyChart messages within RN scope to resolve on first contact when possible.Support medication refills per established protocols.Provide pre-visit assessment as needed and educate patients and families regarding treatment plans.Provide initial patient education and follow-up reinforcement at scheduled appointments.Monitor devices and consult licensed providers as indicated.Coordinate patient referrals, appointments, testing, and admissions to other facilities.Contact patients to relay test results and care instructions as ordered by physicians/APPs.Oversee and assist with in-clinic medications and procedures as necessary.Perform Medicare Annual Wellness Visits.Meet expectations in established service line metrics.Manage patient populations requiring care management support, including assessing healthcare and psychosocial needs and coordinating resources and education.Utilize decision support tools for symptom-based and emergent care.Ensure timely and compliant documentation and billing.Perform other duties as assigned.QualificationsGraduation from an accredited school of nursing approved by the Washington State Board of Nursing. BSN preferred.Current Washington RN license required.AHA BLS for healthcare providers required.Minimum 3 years'' experience as a Registered Nurse required.Preferred hospital-based experience in a specialty area.Applies knowledge of RN scope and role; demonstrates independent critical thinking, problem solving, and decision-making abilities.Establishes and maintains effective relationships with health care team members, patients, families, and outside care providers.Ability to communicate and work effectively with patients across age groups; uses interpersonal communication to achieve outcomes.Aware of patient and family rights and safety standards; adheres to safety and regulatory requirements.Shows financial awareness and commitment to the organizations viability; adaptable to multiple demands; demonstrates accountability for practice and professional development.Seniorities/Employment details: Seniority level Mid-Senior level; Employment type Full-time; Job function Health Care Provider; Industries Hospitals and Health Care. #J-18808-Ljbffr
