In this rewarding role, you will work alongside older people with higher clinical needs, supporting them to live well and maintain their independence while receiving government-funded aged care services. Coordinate and monitor services to ensure they meet clinical goals and client needs. Work collaboratively with GPs, allied health professionals, support workers, and families. Support clients with transitions between care settings and future planning, including emergency preparedness. Maintain accurate, timely, and audit-ready records and documentation. Promote reablement and restorative care strategies that reduce service dependency. Identify risks and ensure safe clinical practice in line with governance standards. Provide weekend and public holiday on-call support on a rotating roster. About You You are an empathetic, motivated professional with a passion for helping older people live independently and achieve the best quality of life. You thrive in a team environment, can manage complex cases, and balance compassion with strong clinical skills. Bachelors degree in nursing or occupational therapy (or similar community-based clinical qualification). Previous experience in community health, aged care, or allied health. Strong understanding of aged care reforms, Support at Home framework, and clinical governance. Experience in assessment, case management, and person-centred care planning. Excellent communication skills with the ability to engage diverse stakeholders. Current First Aid Certificate, drivers licence, and own insured vehicle.
