Pharmacist - Slade Pharmacy, St. Andrew''s Toowoomba Hospital 2 days ago Be among the first 25 applicants Pharmacy QLD - Other Part Time Slade Pharmacy About The Role You will be working across both retail and hospital pharmacy settings, delivering high-quality pharmacy services to retail customers and hospital patients alike. Key Responsibilities Provide efficient clinical pharmacy services to a clinical area. Conduct medication history interviews, medication reconciliation and pharmaceutical reviews at transfers of care. Efficiently dispense and distribute medications. Conduct therapeutic drug monitoring, monitor medication outcomes and adverse drug reaction management. Provide medication counselling and education to patients. Work cooperatively with multidisciplinary teams and provide advice about patient care. Contribute to training and educational sessions for pharmacy, nursing, medical staff and allied health professionals. Support the quality use of medicines by participating in projects, audits, working groups or committees as required. Consider financial implications of medication use in hospital and for patients obtaining medicines in the community. Contribute to on-call and weekend services. About You You are a registered Pharmacist with at least 1 year of practice experience, ready to take the next step in your career. Hospital experience isnt required as comprehensive training will be provided to support your transition.
