Express your interest today and help us shape the future of care. About the Role As a Case Manager, youll support clients across Newcastle/Hunter/Maitland with care thats truly personal. Youll work closely with them, their families and healthcare professionals to create tailored plans that fit their lives, goals, and routines. Its all about helping people stay independent and feel their bestwhether its something big or one of those little wins that mean everything. Were looking for someone who is passionate about health and wellbeing, excels at building relationships, and thrives both independently and as part of a team.
