The Role Youll conduct autonomous, person-centred assessments to determine the care and support older Australians need to live safely and independently at home. Key Responsibilities: Conduct timely, holistic assessments that enhance independence and quality of life. Liaise with families, carers, and service providers as key stakeholders in the assessment process. Manage your assessment schedule to meet Department of Health timelines and quality standards. Maintain accurate documentation and reporting. Contribute to a culture of continuous improvement and exceptional service delivery. Note: This role involves travel within the Gippsland region. Its ideal for someone who enjoys autonomy and time on the road - perfect for catching up on podcasts or audiobooks during your day! What Were Looking For Minimum 1 year of recent RAS/ACAS or SAS experience (essential). SOA2 or SOA4 certification (Comprehensive Assessor transition training provided if you hold SOA2). Tertiary qualification in Nursing, Occupational Therapy, Physiotherapy, Allied Health, or Social Work Strong knowledge of aged care legislation, CHSP, and Home Care Packages. Excellent communication, documentation, and interpersonal skills. Valid Australian drivers licence and a fully insured vehicle. Triple Covid vaccination and 2025 flu vaccination.
