We provide person-centred and holistic support services tailored to assist participants in living safely and comfortably at home by building capacity and giving them choice and control.What we are looking forAt least 1 years experience in the field and registration with AHPRAExperience in CHSP or NDISExperience with outcome measures: DEMMI, PMS, TUG, 6mwt, VASExperience in delivering services to geriatric and adult patientsStrong interpersonal and communication skillsAbility to work independently and collaboratively with a multidisciplinary teamAged Care experience preferred but not a necessityDrivers licence and access to vehicle with comprehensive insuranceWhat we offerSalary range $85,000-$110,000, dependent on experience + SuperCompany CarIn house Supervision provided by our Snr Physiotherapist ( with 10 years experience)Travel to clients within the local area onlyAutonomy to build a case load based on your skillsetFlexible working conditions- Mon- Fri operating business hoursTech savvy company- new laptop and mobile phone providedAbout UsEstablished in 2019, we began with a small but dedicated team, united by a shared vision: to be a guiding light for people with disabilities, helping them navigate the complexities of the NDIS with ease.
