Job Title

Homelessness Response & Intake Practitioner Deliver comprehensive case management and intensive, transitional support to young people, single adults, and families experiencing homelessness. Conduct holistic, strengthbased assessments to develop tailored, goaloriented plans focusing on longterm housing stability and independent living. Provide expert care coordination, service navigation, and referral to connect clients with mainstream and specialist services (health, mental health, AOD, etc.). Offer targeted, flexible, and rapidly responsive support to clients with increased vulnerabilities and complex needs (e.g., trauma, multiple health issues). Work collaboratively with other agencies, participate in community networking, and convene multidisciplinary team meetings to ensure integrated client support. Deliver the Warragul Initial Assessment & Planning (IAP) program two days a week, providing an immediate crisis response and ongoing support to individuals experiencing or at risk of homelessness to establish and maintain their housing.
